I'll hope the misdiagnosis isn't missing a serious illness, and even then since I know up front what the catch is I could just get a second opinion every time. Unless no matter what I'll never get a correct diagnosis. But still, I'll hope whatever I have isn't serious and take the money.

A million dollars but you no longer need to eat or drink, and also can not consume any food or drink either. Feels simple but I imagine it could become really torturous to never eat or drink again. Also have fun taking pills without water.

At first no because of the joys of eating and drinking, but then that's saving so much on not needing to. Hard choice, I mean dang I'd wanna have pasta or Chinese yet I have to say goodbye to them? Too bad it's not possible in reality I suppose I'd say yes but....good food man! So in final nah.

Even as a night owl, I wouldn't take that. I enjoy being awake at night, but not out at night: your options then are pretty much restricted to night clubs, which I don't enjoy!
